Billing Process

As a provider for Directions EAP, your fees and payments will be based on the contracted rate at the time of inclusion in our network.

Providers must submit the following documents in order for us to complete the billing process:

  • Client Information Sheet
  • Personal Office Invoice or HCFA 1500

The above mentioned forms and information must be received by Directions EAP within 60 days of the initial assessment or payment will be denied. Billing forms are used for utilization reporting, and it is very important that these are returned to us in timely manner.

Providers can expect payment within 30 days of submitting the completed paperwork. Directions EAP will not pay providers until the proper paperwork has been submitted.

Below you will find a selection of forms and instructions important to the Directions EAP’s process of working with our businesses and their employees.

Client Information Sheet -EAP client fills this out on first visit and it should be sent with first billing.

Privacy Notice – EAP client should receive a copy of this on first visit.

Client Satisfaction Survey – a copy of survey should be given to client or be instructed to complete on-line on our web-site.

Authorization For Release of Information – to be used in Supervisory Referrals when client sessions must be case managed with us.

Closing Form – needs to be enclosed with final EAP session and/or billing.
